How to fill out the Worksheet (AF Form 469 is ASIMS-generated)

The AF Form 469, Duty Limiting Condition Report, is how a medical provider tells the commander about duty restrictions (DR), mobility restrictions (MR), and fitness restrictions and fitness assessment exemptions (FAE). It is an electronic form, stocked and issued by the medical treatment facility and generated only in ASIMS, so there is no blank for members to fill in. This worksheet helps the member and supervisor capture what the 469 says and manage it. When: When your provider tells you a condition limits your duty, deployment, or fitness test, when the unit receives an ASIMS notification, and before the expiration date so you can get it renewed or closed.

Step by step

  1. Get the finalized 469 from ASIMS or your UDM/UFPM and copy the restrictions and dates here.
  2. Note whether it restricts mobility. Mobility-restricting 469s need the commander's signature before release to the Airman.
  3. Record which fitness components are exempt and until when.
  4. List what the member can still do safely, agreed with the supervisor, without adding medical details.
  5. Track follow-up appointments and the expiration date; a 469 can't run more than 365 days without the special reviews AFI 48-133 requires.

What a strong one looks like

  • An Airman can have only one active 469 at a time, but ASIMS can track several conditions on it with separate expiration dates.
  • A 469 isn't required for conditions expected to resolve within 72 hours.
  • Mobility restrictions of 90 days or less are "light duty" profiles and get monthly AMRO board review.
  • A commander who disagrees with a mobility restriction must contact the MTF/SGP through ASIMS within seven duty days.

Common mistakes

  • Writing the diagnosis. Supervisors get restrictions, not medical details; keep this worksheet the same way.
  • Letting a profile expire before a scheduled PFA. Get it renewed or closed before the test.
  • Assuming a 469 from the last base doesn't count. Fitness restrictions and exemptions stay valid at the gaining installation.
