How to fill out the Worksheet (DAFI 36-2606 reenlistment planning)

There's no single official career-intentions form. This worksheet pulls together what your supervisor, commander, and career advisor will ask before you reenlist, extend, retrain, or separate, using the DAFI 36-2606 process (SRP, CJR, SRB). When: Start 15–18 months before your date of separation (DOS). Under the May 2024 DAFI 36-2606 update, eligible Airmen and Guardians may reenlist up to 12 months before their expiration of term of service.

Step by step

  1. Write down your key dates: DOS/ETS, TAFMSD, and when you enter your reenlistment window.
  2. Check your SRP status (your commander's selection or non-selection on the AF Form 418) and, if you're a first-term Airman, your Career Job Reservation.
  3. Look up whether your AFSC and zone pay a Selective Retention Bonus.
  4. List the options you're weighing and what each needs (retraining quotas, PME, retainability for an assignment).
  5. Take this to your supervisor and career assistance advisor, then make the application in myFSS/vMPF.

What a strong one looks like

  • First-term Airmen need an approved CJR before they can reenlist.
  • If you're not selected under SRP, you have 3 calendar days to state your appeal intent and 10 calendar days to submit the appeal.
  • Retraining, special duty, and assignments often need retainability; reenlisting or extending may be required first.
  • Bonus zones and amounts change; check the current SRB listing before you decide on timing.

Common mistakes

  • Waiting until the last few months and losing a CJR, retraining quota, or bonus window.
  • Separating without a TAP start at least 365 days out.
  • Assuming an extension and a reenlistment have the same effect on bonuses. They don't; ask your MPF.
