How to fill out the DAF Form 77
The DAF Form 77 documents performance for a period or duty an evaluation doesn't cover well: a deployed commander's tour (mandatory), an optional LOE for other deployed or TDY duty, or an acquisition/functional examiner or Air/Space Force advisor review. The AF Form 77 was replaced by the DAF Form 77 (8 Jan 2024). When: Mandatory for deployed squadron, group, or wing commanders who held the position 45 days or more on G-series orders (submit within seven calendar days after the ratee departs). Optional at the evaluator's discretion for other deployed or detached duty.
Step by step
- Complete Section I: name, grade, DAFSC/DSFSC, duty title, and the deployed location or named operation.
- Complete Section II: the type of report and period; for deployed CC LOEs, the level of command, days in the position, and the G-series order.
- For deployed commanders, mark Section III (satisfactorily completed the command tour). A "No" makes it a referral.
- Write Section IV: the impact on mission accomplishment, in performance-statement style.
- The rater completes Section V; for deployed CC LOEs, the additional rater completes Section VI. The ratee acknowledges in Section VII.
- If the LOE contains referral comments, complete Section VIII (and IX if the additional rater referred it) and give the ratee the rebuttal window.
What a strong one looks like
- Lead Section IV with the biggest mission result and quantify scope: sorties, personnel, dollars, missions supported.
- Keep comments on performance you observed during the LOE period only.
- Optional LOEs are informal unless the rater writes a performance report; if the comments warrant permanent recording, AFI 36-2406 says to prepare a performance report.
Common mistakes
- Missing the seven-day suspense for mandatory deployed CC LOEs.
- Referral language without completing Section VIII and giving the ratee 10 calendar days to respond (30 for non-EAD members).
- Entering a real SSN on a draft. Leave the SSN blocks for the official form.
